For most clinics in Dubai, a patient who calls is roughly 6 times more likely to book than a patient who fills out a contact form. A call is high-intent. The patient already decided they want to talk to a human. They are halfway to a booking before you say hello.
Most Google Ads accounts I look at are quietly throttling phone calls without realising it.
Where phone calls leak out of a Google Ads account
The call extension is not turned on
I still find clinic accounts in Dubai with no call extension live. Every search ad should have a call extension active during clinic opening hours. If yours does not, you are leaving 10 to 20 percent of potential bookings on the table for free.
The number is not click-to-call on mobile
On the landing page, the phone number sits in the header as text, not as a tap-to-call link. On a mobile, the patient has to copy the number, switch apps, paste it. About half of them give up.
Wrap every visible phone number in a tel: link. Then verify it actually triggers a call when you tap it.
Bidding is set to Maximise Conversions but call conversions are not counted
This is the silent killer. The campaign is using a Smart Bidding strategy that optimises for conversions. Conversions are defined as form fills and WhatsApp clicks. Calls are not.
Smart Bidding sees that calls are not a conversion. It quietly stops bidding on the hours and devices that produce calls. The phone goes quiet. Nobody knows why.
Add Phone Calls from Ads as a conversion action. Set up call conversion tracking. Tell Smart Bidding what you actually care about.
Mobile bid is too low
Mobile is where 80 percent of healthcare calls in Dubai happen. If your mobile bid adjustment is anything below 0 percent, you are starving the device that produces the most bookings. This used to be common in older accounts that were optimised against form fills in the early 2020s.
Two things that help even more
Call hours match clinic hours
Run a separate ad schedule for the times someone is in the clinic to answer the phone. There is no point paying for calls at 11pm that go to voicemail. The next morning, that patient has already booked somewhere else.
If your clinic is closed, route the ad to a WhatsApp number that the on-call team monitors. Do not let high-intent calls land in voicemail.
Call duration as a quality signal
Set the call conversion to fire only on calls over 60 seconds. A 12-second call is usually a wrong number, a hangup, or a quick rate question. A 60-second call is almost always a real patient.
